BIENNIAL RATE APPLICATION REQUEST REQUIREMENTS

PROVIDER FILING TIMELINES BY PROVIDER NUMBER

The CDSS has established a biennial rate application schedule in accordance with regulations adopted by the California Secretary of State on January 12, 2006. Below is a chart to identify filing due dates, and links to a list of providers who are required to file for each due date. Providers will receive a courtesy reminder letter when their application is due. However, it is ultimately the provider’s responsibility to become familiar with their biennial filing due date and to file when their application is due, according to the schedule.

NOTE: If your provider number is listed under the “End Fiscal Year Unknown” category, you must contact your rates analyst to inform them of your fiscal year reporting period.
